Allergic Conjunctivitis is an inflammation of the conjunctiva of the eye which is the layer of tissue which lines the front part of the eyeball (the white part of the eye) and the inside of the eyelids. While conjunctivitis can be caused by bacteria, virus, or chemical irritation, allergic conjunctivitis is caused by allergies.

According to ASCIA (2019), symptoms may include:

           - Redness in both eyes.

           - Itching and burning of both the eye and surrounding tissues.

           - Watery discharge, often accompanied by acute discomfort in bright sunlight light (photophobia).

           - The conjunctiva itself may become very swollen and may interfere with clarity of vision.

These symptoms can be perennial in nature (all year long) or seasonal (present in various seasons).

Angioedema is a condition in which small blood vessels leak fluid into the deep dermal, subcutaneous and/or submucosal tissue, causing swelling. The swelling usually occurs around the face, lips and tongue but can also affect the throat, hands, feet and genital area.

Angioedema is most frequently related to the administration of ace (angiotensin converting enzyme) inhibitor blood pressure medications. Although allergy is a very rare cause of Angioedema, it can still be triggered by food or drug allergies. Usually if the cause is allergy related, angioedema and hives will occur simultaneously.

An episode of angioedema typically lasts between one and three days, however can persist for weeks. Allergy-induced angioedema may be suspected if the swelling is rapid in onset but resolves within 24 hours. Swelling which persists for days or weeks is rarely caused by allergy.

Angioedema is not usually dangerous, and in general, most mild to moderate symptoms can be sufficiently treated with anti-inflammatory medications. Severe swelling of the throat or tongue is a medical emergency due to the risk for breathing difficulties and airway obstruction. Individuals should seek immediate medical attention.

Dust is omnipresent! it is everywhere all of the time. It is found indoors and outdoors, within households, workplaces, schools, cars and public places. Dust is made up of virtually anything and everything. Dust contain human skin cells, insect fragments and waste (e.g. dust mite or cockroach), animal dander, pollen, mould spores, food debris, paper, dirt, sand, minerals from outdoor soil, and many more.

Every household has its own specific mix of dust. It is a normal by-product of the breakdown of fibres and other substances found within a living environment. Since it is impossible to completely eliminate dust it is important to understand that there are many substances embedded within dust that can trigger allergic reactions. Dust mites are the most common cause of allergy from dust. Other causes include cockroaches, mould, pollen and animal dander.

When it rains, or is humid, pollen grains can absorb moisture and burst open, releasing hundreds of small pollen allergen particles that can enter the small airways of the lungs (Ascia, 2019). It is important to mention that although not all individuals effected by this phenomenon suffer from Allergic Rhinitis, there is a strong association between the two. In addition since about 1 in 4 individuals suffering from Allergic Rhinitis also suffer from Asthma it is imperative to manage allergic rhinitis symptoms to avoid potential complications from thunderstrom Asthma.

The aim of a low salicylate diet is to keep the intake of salicylates to a minimum. Salicylates occur naturally in many fruits vegetables, nuts, herbs, spices, jams, honey, yeast extracts, tea, coffee, juices, beer, wine and natural flavourings (e.g. peppermint). Salicylates are also frequently used in medications, perfumes, botanical oils and toothpastes.

Important considerations when undertaking a diet low in salicylates:

Salicylate is highest in unripe fruit, and lowest in fruit ready to drop off the plant

Salicylate is highest in the skin zone of fruit and vegetables (skin and just below skin)

Salicylate is concentrated by processing such as within fruit or vegetable juices, sauces, pastes, powders, jams, syrups and flavourings

Salicylate is increased in genetically engineered plants for greater resistance to disease

Natural chemicals are found in the foods we eat. Food is composed of protein, carbohydrate, fat and various nutrients as well as a number of natural chemicals. These naturally occurring chemicals often add flavour and smell to food. Sometimes they will trigger adverse symptoms in unlucky individuals.

Salicylates are one family of chemicals that occur naturally in plants. They act as a natural pesticide, and protect plants against harmful bacteria, insects, fungi and disease. Salicylates are present in fruits, vegetables, nuts, herbs, spices, jams, honey, yeast extracts, tea, coffee, juices, beer, wine and natural flavourings (e.g. peppermint). Salicylates are also created synthetically and can be found in perfumes, scented toiletries, cleaning products, washing powders, botanical oils (e.g. lavender, eucalyptus and tea-tree) and medications (e.g. aspirin).

Salicylates have a cumulative effect in the body and build up over time. The tolerated amount varies from person to person. In high enough doses salicylates are harmful to everyone, however most people can handle average amounts without experiencing adverse effects. People with salicylate sensitivity are unable to ingest or inhale a normal amount of salicylate without experiencing adverse effects.

Symptoms of salicylate sensitivity vary but typically include headaches, migraines, eczema, hives, gastro-intestinal irritation, rhinitis, nasal polyps and exacerbation of asthma. Salicylate sensitivity is also associated with behavioural problems in children, sleep disturbance, anxiety and depression.

It is important to recognise that salicylate sensitivity is not an allergy, but an intolerance. There are no reliable skin or blood tests for the diagnosis of food intolerance. The only way to accurately diagnose a salicylate sensitivity is through a process of dietary elimination and challenge testing. This should always be done under the supervision of a doctor and dietician. Upon diagnosis, the easiest way to treat salicylate sensitivity is by limiting salicylate consumption with a low salicylate diet.

           What is pollen?

Pollen is a very fine powder that is produced and released by plants (trees, weeds, flowers, grasses) to fertilize other plants of the same species. Pollen is typically dispersed in the wind, but can also be transported from plant to plant by birds and insects.

           What is a pollen allergy?

Pollen consists of tiny particles which, when inhaled, can trigger symptoms of allergic rhinitis (hay fever) and exacerbate asthma in susceptible individuals. Common symptoms of allergic rhinitis and asthma include:

            - Sneezing

            – Increased mucous production (i.e. a runny nose)

            – Nasal itchiness – Post-nasal drip

            – Itchy, red, watery eyes

            – Nasal congestion (i.e. blocked nose)

            – Coughing – Wheezing

            – Chest tightness

           – Shortness of breath

Pollen is one of the most common triggers of seasonal allergies. A seasonal allergy is where individuals only experience allergic symptoms for the period or season, the pollen to which they are allergic, is in the air. Wind-borne pollen from trees, grasses and weeds, tend to be more highly allergenic and the cause of most allergy problems. However, not all pollen allergies are the same, nor are they restricted to only cause allergic symptoms during spring.

There are countless different types of pollen, and the timing and duration of pollen seasons differ depending on the plant varieties involved. In Australia grasses tend to pollinate during spring and summer, trees in late winter and early spring, and weeds from August through to May. Thus, depending on the pollen allergen or allergens involved, the severity of pollen allergies can vary considerably from one individual to another.

           What is Eczema?

Eczema is a chronic, non-infectious, inflammatory skin condition, which affects approximately one third of the Australian population at some point in life. Eczema causes patches of the skin to become red, itchy, dry and scaly. In some cases, the skin may weep. Eczema is usually found in the creases of the elbows, behind the knees, across the ankles, and may also involve the wrists, face, ears and neck.

           Who does eczema affect?

Eczema most commonly occurs in infants and young children, affecting one in five children under two years of age. Symptoms of eczema usually improve with age, however many children will continue to have a tendency for dry, sensitive skin, through to adulthood. Eczema is not frequently observed in older children and adults

           What causes eczema?

The cause of eczema is not well understood. In most cases, eczema is thought to be a hereditary condition. A strong association between eczema and allergic disorders (such as asthma, hay fever and food allergies) has been identified in the literature. However it is important to understand that eczema is not an allergic reaction, so an allergy to a particular substance will not cause an individual to develop eczema.

Please note these are examples of potential triggers of eczema, and it is not routinely recommended individuals with eczema avoid all of these triggers without seeking the advice of a qualified health professional.

As there currently remains no cure for eczema, the focus of eczema treatment is prevention and management of symptoms:

Maintain and protect the skin The use of emollients is essential to keep dry skin moisturized and minimize itchiness.

Promptly treat eczema flare-ups It is important to eczema flare-ups are treated quickly with topical steroids or calcineurin inhibitors. These medications act to reduce skin inflammation. Individuals should also continue applying emollients to keep the skin moisturised.

Control eczema itch Eczema itchiness can be very distressing. Scratching can cause eczema to worsen and increases risk for infection. To control itchiness, individuals can apply cold compresses or wet wraps. Antihistamines are not usually very effective for eczema itch, but may sometimes help.

Prevent and treat infection Although eczema is not infectious, it can become infected. Eczema compromises skin integrity and impairs the natural barrier function of skin against bacteria. It is important individuals keep their skin well moisturised, and refrain from scratching. Should eczema become infected, it is important it is treated promptly.

Avoid triggers and irritants Identifying and avoiding triggers which aggravate eczema can be very helpful in preventing and/or decreasing the severity of, eczema flare-ups. Common triggers are listed above.

Nasal irrigation is often recommended as a complementary non-pharmacological therapy for the relief of nasal congestion associated with allergic rhinitis (hayfever) and rhinosinusitis. It is a personal hygiene practice in which the nasal and sinus cavities are washed using an isotonic saline solution. Saline nasal irrigation help to relieve inflammation and congestion by flushing away problem allergens, irritants and infectious materials, excess mucous and the inflammatory mediators (such as histamine) it may contain, from the sino-nasal cavities.

When performed regularly, saline irrigation can help improve nasal symptoms, reduce medicine consumption and enhance the quality of life of allergic rhinitis sufferers. For best results, irrigation should be performed twice daily, or as directed by your healthcare professional.

Nasal irrigation using a saline solution is generally well tolerated, inexpensive, easy to perform, and associated with no adverse side effects. Nasal irrigation kits are available from most leading pharmacies throughout Australia. At Allergy First we support and introduce nasal irrigation to most of our allergic rhinitis patients.

Urticaria is the medical term for hives, an allergic-like reaction of the skin, in which areas of the skin become red, itchy and raised. Urticaria is the most common cause of skin inflammation. It is estimated to affect around 1 in 30 children, and up to 1 in 5 people throughout a lifetime. Though distressing, urticaria is generally a benign and self-limiting condition.

In response to a trigger, the immune system activates and the cutaneous mast cells release inflammatory mediators such as histamine. Histamine causes the small blood vessels (capillaries) in the superficial layers of skin to leak fluid, and wheal-like swellings to develop on the surface of the skin. The wheals on the skin can be rounded or flat-topped, and vary in size from that of a pinprick to the size of a dinner plate. They are typically red with a pale centre, and blanch upon applying pressure.

There are two types of urticaria, acute and chronic. Urticaria usually develops quickly (within minutes) and resolves quickly (within hours or days). If episodes of urticaria recurrent, but last less than 6 weeks, than the urticaria is acute. The majority of urticaria presentations are acute. In the circumstance urticaria does not resolve within 6 weeks, it is classified as chronic. Acute urticaria is more common in children; chronic urticaria is more common in adults.

Though an allergic-like reaction, urticaria can be caused by both allergic and non-allergic mechanisms. Allergic triggers can include medications (antibiotics, analgesics, anaesthetic muscle relaxants, non-steroidal anti-inflammatory drugs), insect stings and foods. Physical contact with an allergen (e.g. animal saliva or dander, plants, raw foods, latex) can also cause urticaria. Non-allergic triggers can include infections, changes in body temperature, exposure to cold and exercise. Often the cause of urticaria is not clear.

The treatment of hives predominantly involves avoidance of the triggers which cause and aggravate urticaria. At Allergy First we can help distinguish between allergic and non allergic triggers to urticaria.

All metals in contact with biological systems undergo corrosion and release metal ions which can activate the immune system of susceptible individuals. This is significant as metal-on-metal prostheses are increasingly being used within orthopedic surgeries, and hypersensitivity to metal is far from uncommon. In fact, dermal contact with metals and ingestion of metals is estimated to cause immune reactions in up to 10-15% of the population.

Contact patch testing is increasingly being recognized by orthopedic surgeons as a useful preoperative method for identifying patients who may be at higher risk for experiencing a metal hypersensitivity reaction and complications following joint replacement surgeries.

Being able to breathe through the nose is something many take for granted. Not only does most normal breathing takes place through the nasal cavity, nasal breathing is the only physiologically normal breathing pattern. Any obstruction within the nasal cavity can be a source of discomfort and have a significant impact on daily life. Nasal airway obstructions narrow and block the nasal passages and prevent the comfortable flow of air in and out of the nose.

Many allergies can be the cause of nasal airway obstruction through the constant process of inflammation and swelling of the nasal passages and sinuses (e.g. nasal allergies, sinusitis) which at times can even lead to structural abnormalities in the nose (nasal polyps).

There are a number of potentially serious health consequences associated with mouth breathing. When left untreated individuals can develop craniofacial growth abnormalities, dental and orthodontic problems, skeletal and postural changes, sleeping difficulties, exacerbation of asthma and various other physiological and social health issues

Signs of mouth breathing include: Nasal congestion (“blocked” nose), intermittent or constant open mouthed breathing, noisy, visible breathing (physiologically normal breathing is through the nose, silent, smooth and almost invisible), snoring, frequent night waking, dry mouth (often individual’s report needing to keep a glass of water by the bedside), dry, cracked lips, night sweating, daytime fatigue, behavioural issues (in children), long, narrow face, dental or orthodontic issues, head posture where head it tilted backwards, bad breath, and poor asthma control.

Asthma is defined as chronic inflammatory disorder of the airways, characterised by reversible airway narrowing with obstruction of airflow to the lungs. Asthma is very common, affecting 1 in 4 children, 1 in 7 teenagers and 1 in 10 adults in Australia.

Individuals with asthma have hyper-responsive airways, which overreact to stimuli that do not affect other people. During an acute episode of asthma or an “asthma attack” exposure to a trigger causes three main changes to occur within the airways: the lining of the airways become inflamed and swollen, the thin layer of muscle within the wall of the airways constrict and excessive mucous is produced. The airway narrows, and causes individuals to experience one or more of the following symptoms.

Allergens are by far the most common of asthma triggers, with over 80% of asthma sufferers having underlying allergies. Allergic asthma is usually caused by the inhalation of such aeroallergens as dust mites, pollens, moulds and animal dander. Food allergies can also stimulate the onset of asthma symptoms in susceptible individuals.

Moulds are microscopic fungi made up of clusters of filaments, and live on organic (plant or animal) matter, which they decompose for their nourishment. Moulds are among the most widespread living organism, with many different varieties. Most mould reproduce by releasing spores into the air, which then settle on organic matter and grow into new mould clusters. These airborne mould spores are far more numerous than pollen grains, and when inhaled can produce allergic rhinitis symptoms in susceptible individuals. Moulds can be found both indoors and outdoors. Unlike pollens, moulds do not have a limited season. Moulds flourish in damp, dark and warm areas where there is poor ventilation. Mould spores produced outside are widely dispersed in the air, and can enter the home. Other moulds are produced within the home.

Cat allergy is the most common pet allergy. The cat "allergen" is produced and found mainly in cat saliva and skin flakes. As part of their grooming process, cats deposit the allergen on their fur when licking themselves. The cat allergen binds itself to hairs, dust particles and around the home. The allergens become airborne as microscopic particles which when inhaled can produce allergic symptoms.

The cat "allergen" can remain within soft furnishings, such as sofas, carpet...s, rugs, bed mattresses even after the cat has been removed from the home. Cat allergens can persist within the furnishings up to 5 years.

Hay Fever, also known as allergic rhinitis, is an IgE-mediated inflammatory disorder of the nasal passages caused by the overreaction of the body’s immune system to inhaled allergens.

Airborne allergens such as pollens, animal dander, mould and mites, interact with specific IgE antibodies on the surface of mast cells within the nasal mucosa. This interaction triggers the mast cells to release pro-inflammatory mediators such as histamine, which cause inflammation and swelling of the nasal mucosa.

Symptoms of allergic rhinitis can develop within minutes of inhaling an allergen. Common immediate symptoms include sneezing, runny nose, nasal itchiness, a post-nasal drip and itchy, red, watery eyes. Over time, individuals often develop nasal congestion and possibly nasal polyps due to chronic inflammation of the nasal passages.

Allergic rhinitis is classified as intermittent or persistent, mild, moderate or severe. Intermittent allergic rhinitis is when an individual experiences symptoms for four or less days per week, and/or for less than four consecutive weeks at a time. Persistent allergic rhinitis is when symptoms occur more than 4 days per week, for more than 4 consecutive weeks.

Also taken into consideration when determining the severity of allergic rhinitis is the impact an individual’s symptoms have on their quality of life. That is, the degree to which an individual’s allergic rhinitis affects their ability to sleep, carry out activities of daily living, and go to work or to school.

House dust mites are the most common cause of dust-related allergy. House dust mites are microscopic eight-legged creatures which are part of the arachnid family (relatives of spiders and ticks). They get their name from their habitat – household dust. Human living conditions are favourable to house dust mites because humans provide a source of both humidity and food. House dust mites thrive in warm, moist environments, and feed on the dead skin cells of humans (and their pet...s) found within dust. Exposure to house dust mites is a major source of allergy throughout the world. It is not the presence of the mites themselves which triggers allergy though. The house dust mite allergen is a protein which is contained within mite faecal matter and secretions.

House dust mite allergy is the most commonly encountered dust-related allergy. It is the leading cause of perennial allergic rhinitis (persistent hayfever), and a primary trigger for asthma exacerbation, affecting up to 85% of asthmatics. Contact with house dust mite allergens has also been associated with inflammation of the skin and worsening of atopic dermatitis (eczema).

House dust mites are present all year round and survive in all climates. They are found within most homes and are difficult to eliminate and avoid. House dust mite populations are usually highest within the bedroom – on mattresses, pillows and bed linen. Upholstered furniture, carpet, rugs, curtains and soft toys are other common places house dust mites densely inhabit.

Skin prick testing (SPT) is a major diagnostic tool in the field of allergy. SPT is recommended as a primary method for the identification of allergic sensitivities in such conditions as allergic rhinitis, conjunctivitis and asthma, atopic dermatitis, and food related acute urticaria, eczema and anaphylaxis.

SPT involves the introduction of small amounts of suspected allergens onto and just below the surface of the skin. A small, superficial break in the skin is made with prickers which have been dipped in allergen extracts. The skin is then closely observed for 15 minutes for signs of a wheal and flare reaction. SPT has a high sensitivity and specificity for the diagnosis of allergic sensitivities. It is well tolerated, carries low risk and delivers rapid results.

Food allergy and food intolerance are commonly confused because the symptoms of food intolerance occasionally resemble the symptoms of food allergy. However there are important differences between the two.

A food allergy is an overreaction of the body’s immune system to an ordinarily harmless component within a food. The food is perceived as “foreign” and therefore threatening, so the immune system produces allergen-specific immunoglobulin E (IgE) antibodies to protect the body. In response to an “infestation” the IgE antibodies bind to food allergens; triggering the release of histamine from mast cells and an immediate hypersensitivity reaction.

A food intolerance is a chemical reaction rather than an IgE-mediated immune response. Chemicals within food cause reactions by irritating nerve endings throughout the body. Unlike a food allergy though, a food intolerance cannot illicit a severe allergic reaction, or anaphylaxis.

It can be difficult to tell the difference between the symptoms of food allergy and food intolerance. In general, symptoms caused by a food allergy develop very quickly after consuming the offending food. Symptoms of food intolerance can be immediate, but usually develop after 12-24 hours. Furthermore, unlike with food allergy reactions, food intolerance reactions are usually related to the amount of food consumed. An individual with food intolerance may be able to eat small amounts of the offending food without experiencing symptoms.

            What is Immunotherapy?

Allergen immunotherapy or "desensitisation", is considered the closest thing to a “cure” we currently have for allergies. Immunotherapy is like a vaccine. Treatment involves the regular administration of gradually increasing doses of allergen extracts over a period years. Over time, the body develops immunity and the allergens can be tolerated with few or no symptoms.

Immunotherapy is the only treatment available that targets the cause of allergy rather than just the symptoms. Immunotherapy “switches off” the allergy, by changing the way the immune system reacts to offending allergens. Immunotherapy:

            • reduces the production of the allergen-specific IgE antibodies which initiate allergic reactions

           • moderates the production of inflammatory mediators (such as histamine) which cause allergic symptoms • stimulates the production of blocking antibodies (antibodies which block the allergen-specific IgE antibodies from initiating allergic reactions)

Allergic contact dermatitis is a skin condition in which direct contact with an allergen triggers a cell mediated allergic response and the development of localised itchy, red rashes on the skin. The rashes typically present a few days after contact with the allergen and persist for as long as there is contact with the allergen. It can take several weeks for rashes to disappear and the skin to heal after contact with the allergen ceases.

Hay fever now affects 3 million Australians. This means that 1 in 5 of us have to battle through spring and summer with runny noses, watery eyes, and the other symptoms of hay fever.

On top of that, Asthma Australia reports 1 in 10 Australians now suffer from asthma, and around a third of these people report having asthma symptoms that interfere with their daily lives. In Australia, hay fever and asthma are both on the rise. This isn’t really surprising as the same things can trigger both conditions; typically dust mite, pet allergens, pollen and moulds. What also should come as no surprise is the fact that asthma and hay fever are often found in the same patients. In fact, most patients with asthma also have hay fever, and hay fever is one of the biggest risk factors for developing asthma.
